What to expect:

If you have any questions about physical therapy or want to determine if our services are right for you, please book a discovery call. During the call, we can discuss your ailments or injuries and address any questions you may have before the evaluation.

Before your appointment, you will receive an email with intake forms to complete. These forms include sections for your past medical history, the reason for your appointment, ect. Completing these forms in advance allows our physical therapist to prepare and make the most of the time allotted for your appointment.

Physical Therapy Evaluation

The initial evaluation aims to assess your current condition and determine the causes of your symptoms. This will include discussing the details of your pain, injury, or surgery as well as completing a hands-on examination. Treatment typically begins during this evaluation so we can initiate the rehab process as soon as possible. Based on the information gathered on your evaluation, we will recommend the frequency of sessions needed to achieve your desired outcomes. As a team, we will discuss what fits into your schedule, budget, and life.

Payment Options

Insurance

We are currently in the process of becoming in-network with Minnesota insurance companies. We are happy to call on your behalf to check out-of-network costs for you. Please call us if you have further questions regarding insurance coverage.

Self-Pay

If you prefer to pay out of pocket using funds from your HSA, credit card, or cash, we offer the following pricing for Physical therapy services:

Physical Therapy Evaluation: $140
Physical Therapy Follow-Up (45 min): $125
Physical Therapy Follow-Up (30 min): $99

If you wish to submit a claim to your insurance company for reimbursement, you will be provided with proof of payment, and a super bill that includes necessary and appropriate codes, as well as supporting documentation requested by your insurance company.
